UCI LICENCE APPLICATION 2008

Dear all,

The UCI licence application is now really for enrolment. We are pleased to inform you that the fees stay the same at S$100 for this year, but do take note of the additional charges that may apply.

Please download the PDF document, print and fill up the details accordingly.

You are reminded to:

1. Read carefully the instruction.

2. Provide all required details in all sections of the Application Form.

3. Sign the Declaration.

Please return the fully completed Application Form to:

c/o Tanglin P O Box 562, Singapore 912419

together with the following items:

1. Two (2) colour passport sized photographs with white background.

2. Copy of passport or NRIC for identification purposes.

3. Fees payable by crossed cheque in Singapore dollars and made payable to Singapore Amateur Cycling Association.

Do take note that if all information is not provided in the Application Form, the application will not be processed and SACA and its membership coordinator will not be responsible for any delay in the issuance of the UCI Licence.
